Are the same person, I think. One's her maiden name, and one's her married name. Unless I have some drastic misunderstanding of the character from not having seen the fourth film.

I don't see duplications to other fandoms on your list of okay topics, but in case they are:

In this year's round of Marvel Fandom Wackamole, all of the New Mutants (Comics) characters, and all of the Guardians of the Galaxy (Film) characters have duplicated into Marvel 616.

All of the Doctor Who (1963) characters have duplicated into Doctor Who (Big Finish), but they are all in both, so I'm not sure where they originated.

"Turner Series- Courtney Milan" and "Perfect Creature" are both under "Uncategorized Fandoms." The Turner series is a series of books, and Perfect Creature is a movie. Also, Zuleika from "The Steel Seraglio" seems to have gotten attached to a character of the same name from Joseph King of Dreams- they actually don't have anything to do with each other.

I'll second the question about Darla being listed in both Buffy and Angel. Seems like once would be enough (in Angel).

Also, I nominated the "potential slayers" in Buffy and it was rejected, though it is an existing tag. I know there aren't enough fics about all of them put together, so I'm curious about the reasoning. Is it because it's cheating to nom them all in a lump — and thereby getting a whole bunch of rare women in one fell swoop — or something else? It just seemed silly to nominate just one or two of them, since they don't get much love. But any of these would be great:

Vi, Rona, Annabelle, Chloe, Amanda, Chao Ahn, Molly, Caridad, Eve, Shannon, etc.

... I think she's Jeremy Renner's daughter, and is therefore an RPF character? She's definitely not on Defiance.

(There is a Defiance character whose name is similar - Jessica "Berlin" Rainier. She's been nominated and approved.)

Under Toaru Kagaku no Railgun | A Certain Scientific Railgun, Misaka Mikoto and Shirai Kuroko are listed both with disambigs and without. Honestly, I find this puzzling, since it wouldn't let me nominate them without the disambiguations (because they were also nominated under A Certain Magical Index), but I guess it let someone else do it?

Not Mod: they were probably initially nominated under Index, then after you submitted them with disambigs under Railgun, wormed into Index thanks to the "child tags tend to float into every fandom they're associated with by wranglers, even if the tag set mod tried to associate them to only one fandom for that tag set" bug.

Gilmith is in both The Lord of the Rings and the Silmarillion. I THINK she was nominated under LotR.

Cleante al-Faisal, Mara (Star Trek) & T'Shael have jumped from Star Trek - Various Authors to Star Trek TOS.

Under Agents of SHIELD, Agent 33 and Kara Lynn Palamas are the same character. Thus far, she has been referred to as Agent 33 on the show, but according to the Marvel comics the series is based on, her real name is Kara Lynn Palamas.
